AltCareDex is a publisher of fact sheets, which you cite like a pamphlet. The APA citation (for a pamphlet or brochure with no author or place of publication listed) will look like the following example:

Title of brochure [Pamphlet]. (Year). N.P.: Publisher.

N.P. stands for "no place of publication." APA also allows the substitutes n.p. for "no publisher" and n.d. for "no date of publication. APA does not require you to include information about the database, so your citation will look like the following:

Sound therapy [Pamphlet]. (2012). N.P.: AltCareDex.

That was a toughie! Thanks for the challenge. :)
